New Student Registration Forms - The Out of Area School form only needs to be completed if Bayside is not the catchment school, but you are hoping to attend Bayside. The other two forms must be completed for complete registration. Please bring your forms to the Bayside office, along with the student's original Birth Certificate/Passport, Care Card and Proof of Residence (for a complete registration). Registration forms are also available at the Bayside office.
